| POSITION SUMMARY: |
| The Personal Support Workers – Gwen Lee Supportive Housing (PSW-GL enhance the quality of life of SJHC clients by providing quality and compassionate care in the form of personal care assistance, medication reminders, physical and cognitive support and home keeping assistance. PSW-GL assists in enabling individuals to continue living independently in a safe and healthy manner in their own home/apartment units, within the Gwen Lee program. Gwen Lee Supportive Housing clients are typically able to direct their own care, are over the age of 65, require on average 1.5 hours of personal support care each day, and are medically stable. |
