POSITION SUMMARY:
The Occupational Therapist– St. Mary’s Health @Home (OT- SM H@H)  assists clients with existing physical conditions through standard scope of practice activities, OT- SM H@H play a crucial role in helping clients achieve maximum functional capabilities, maintain these capabilities, and prevent future injuries. This position involves following customized rehabilitation programs, educating and monitoring clients, and clearly communicating client progress to others.

This role supports Manager – Therapy Services in the efficient and effective operations of program activities, ensuring the safety and health of clients through infection control, ongoing communication, education, and monitoring activities.

Clients are typically individuals discharged from the hospital but require some continuing medical supports. These clients undergo care assessments, are medically stable, may be of any adult age group, and might require mechanical or two-person lifts. Clients typically require assistance for 21 days or 16 weeks, and they may or may not require a formal care plan.


CORE DUTIES AND RESPONSIBILITIES
Clinical
  • Assess patients to determine strengths and weaknesses cognitively, perceptually, physically, and psycho-emotionally.
  • Determine clients' ability to function in self-care, productivity, and leisure, considering the influence of the environment.
  • Interpret data for discharge planning and develop treatment plans for enhanced functioning and independence.
  • Communicate effectively with the interdisciplinary team using verbal and written formats.
  • Maintain workload statistics and participate in documentation procedures.
  • Educate and communicate progress, therapy plans, exercises, and/or mobility aids to clients.
  • Work with various mobility equipment, providing person-centered support and promoting goal-oriented progress.
  • Facilitate access to community, social, and healthcare services for clients.
  • Set up and prepare therapy equipment and may perform delegated tasks within the scope of practice.
  • Supervise activities of OTA/PTA and/or students on occasion.
  • Facilitate information sessions for clients and/or visitors.
  • working with clients with multiple complex medical conditions in an at-home environment

Safety
  • Practice proper infection control protocols with clients and equipment.
  • Monitor therapy areas, equipment, and mobility devices for defects, reporting safety concerns promptly.
  • Perform daily maintenance and cleaning of therapy equipment and treatment areas.
  • Actively promote client safety and education about safe practices.
  • Participating in organizational program safety programs and protocols (eg. safety checks)
  • Engage in continuous improvement initiatives.
  • Demonstrate knowledge, sensitivity and expertise in the care of seniors and their families

Administration
  • Collaborate with others in ordering / procurement of required supplies.
  • Record treatment activities in client charts, maintaining ongoing communication with interdisciplinary program staff.
  • Develop and present reports and metrics as requested.
  • Support clients in setting up therapy appointments and/or attending group activities.
  • Support referral/intake processes of the program.
  • Provide general assistance to program other Clinical Managers and/or Program employees.
  • Perform general administrative duties related to the role.
  • Participate in committees and program development related to areas of practice.

KEY EDUCAITON AND EXPERIENCE
  • Degree in Occupational Therapy from a recognized university and registration with the
  • College of Occupational Therapists of Ontario, required
  • Masters Degree in Occupational Therapy,  preferred
  • Eligible for membership in OSOT and CAOT, preferred
  • Current BCLS/HCP certification, required
  • 1-3 years experience, required
  • Gentle Persuasive Approach or equivalent training, preferred
  • Previous experience working with clients with multiple complex medical conditions, required
  • Experience working in a Home or Community setting, preferred
  • Ongoing professional development and learning, required
  • Previous experience and proficiency with mobile work tools, devices, and apps, required
