 Position Summary

As a Physiotherapist, you’ll play a vital role in helping clients achieve and maintain their highest level of functional independence. You’ll assess, plan, and implement personalized rehabilitation programs, working closely with clients, families, and healthcare teams to ensure safe, effective, and meaningful care.

This role is ideal for a compassionate, innovative, and collaborative professional who thrives in a dynamic home and community care setting.


Key Responsibilities

  • Conduct client assessments and develop personalized treatment plans
  • Provide education on therapy goals, exercises, and adaptive equipment
  • Collaborate with healthcare teams for care coordination and discharge planning
  • Supervise support staff and mentor students
  • Ensure safety protocols and maintain organized therapy spaces
  • Document care activities and support scheduling and referrals

What You Bring

  • Bachelor’s or Master’s Degree in Physiotherapy (Master’s preferred)
  • Registration with the College of Physiotherapists of Ontario (CPO)
  • Current BCLS/HCP certification
  • 1–3 years of clinical experience (required)
  • Experience with complex medical conditions and mobility support
  • Home and community care experience (preferred)
  • Familiarity with mobile work tools and EMR systems

This position requires driving. Are you willing and able to travel throughout the areas of HNHB boundaries including Hamilton, Burlington, Brantford, Niagara regions and Norfolk County?*
This is a community care position that requires working independently and alone in the clients homes to provide care. Are you comfortable and able to work in this environment?*
How many years of experience do you have working as a Physiotherapist?*
Do you have experience supervising Physiotherapy Assistants (PTA)?*
Do you have experience working in a Home or Community setting?*
Do you have experience working with clients with multiple complex medical conditions in an acute physical medicine / acute mental health environment particularly with older adults?*
Do you have experience working as part of an interdisciplinary team to provide care for clients?*
Do you have experience using mobile tools, devices and equipment required for a community position?*
Are you proficient in writing reports and assessments, measuring workloads and accessing information through excellent computer and MS office skills necessary to be successful in the role?*
Do you continually participate in professional development and training in your profession as a Physiotherapist? If yes, please explain.*
