POSITION SUMMARY: The Supervisor – Care Coordinating, visiting (SCCV) is responsible for the effective coordination of the PA team, assessing patient care paths, monitoring nursing processes and for providing clinical guidance to nurses, in order to ensure the provision of high-quality client care. The SCCV demonstrates effective leadership and advises on best practice, as well as SJHC’s policies and procedures. This role also monitors the delivery of services and scheduling to ensure high productivity, and to maintain efficient operation of the visiting department.
This role directly supports the Clinical Manager – Visiting Program by acting as the point of contact for other Visiting staff members, as it relates to the day to day operations of the program, including scheduling functions. The SCCV also works in close co-operation with external stakeholders and other internal Managers.
CORE DUTIES AND RESPONSIBILITIES Supervision / Operations
Supervising the Visiting Program Assistants in their daily activities, including performance management and scheduling in co-operating with the program Manager
Acting as an initial point of contact for operational questions and/or day to day functionality of the team
Ensuring that the work output of the team members is accurate, efficient and effective
Ensuring team compliance to policies, procedures, regulations, training and external requirements
Acting as the point of contact for the team members, as it relates to interpretation of Collective Agreements and other legal compliance items
Assisting the Clinical Manager – Visiting Program with staff scheduling, including regular shift, vacation and on-call scheduling requirements
Optimizing service utilization to ensure qualified and adequate staffing to facilitate appropriate care in a fiscally responsible manner
Maintaining high levels of team work, integrity, and confidentiality among Visiting clinical and admin staff
Assisting with program staff events such as staff meetings, team activities, improvement initiatives and other team activities as required
Providing training to other program staff members on general operational requirements including processes, procedures and software (eg. Procura)
Ordering supplies for the Visiting team
Facilitating communication between nursing staff, management team members and all customer and client groups
Assisting nurses with daily caseloads and weekly planning
Responding to calls from the LHIN and clients regarding client concerns
Completing LHIN Risk Event investigation and documentation, inclusive of performance management of the employee involved in cooperation with the Clinical Manager
Filling vacant shifts when necessary
Participating in the orientation process of new employees, as required
Actively promoting and participating in the program’s continuous improvement and risk management initiatives
Other duties as required or assigned
Clinical
Participating in joint visits with nurses in the field where required
Performing clinical audits and visual observations
In taking new clients; setting care plans in electronic medical record software, assessing and managing case referrals and nursing assignments based on availability, seniority, nurses’ skills and client needs
Formulating service plans and service requirements with input from the field staff based on best evidence-based practice
Monitoring and re-evaluating client care plans including PEDs, number of visits, frequency changes and clinical care paths (OBP/OBR) as required, with input from the field staff
Assisting nursing staff with developing, initiating and coordinating individualized and or standardized (OBP) care plans for clients, and provide ongoing resource management and discharge planning as appropriate
Conducting record reviews and chart audits as needed
Monitoring client updates on Procura, completing the discharge process in accordance with LHIN standards
Providing input in program interpretation/education to community agency staff, physicians, clients and families

KEY EDUCAITON AND EXPERIENCE
Registered Practical Nurse, or Registered Nurse Diploma, required
3-5 years of nursing experience or care coordinating experience, required
1-3 years of supervisory experience, preferred
Member in good standing with Ontario College of Nurses, required
Advanced knowledge of MS Office, including Excel, Word and Outlook, required
Understanding of electronic medical record and/or other scheduling software, required
Previous experience working in a community/home care/Acute or LTC setting, preferred
